MARYVILLE, ILL. (First Alert 4) - A nursing home in Maryville, Illinois is closing amid allegations of abuse and neglect.

First Alert 4 has obtained a notice from the Centers for Medicare & Medicaid Services (CMS) which was issued April 2, announcing funding would be pulled from Au Well Care Home, Inc. in 30 days.

According to the notice, the facility failed to attain substantial compliance with multiple Medicare and Medicaid participation requirements. The agency cited abuse, neglect, and exploitation, quality of life, and quality of care.

First Alert 4 visited the facility Monday. The building’s front doors were locked. A letter on the door confirmed the closure.

We reached out to the number left on the door to learn more information about what led to the closure and where the residents were transferred.

We are waiting to hear back.
